Please read these Terms and Conditions ("Terms") carefully before using the taggehealth.com website or enrolling in any services offered by Tagge Precision Health, PLLC ("we," "us," or "our"), operated by Dr. Daniel Garth Tagge, M.D. By accessing our website or engaging our services, you agree to be bound by these Terms.
Tagge Precision Health, PLLC is a physician-led virtual concierge precision medicine practice licensed in North Carolina. Our services are focused on preventive health, metabolomic-based optimization, and evidence-based precision medicine.
Our flagship service is Concierge Precision Medicine, an ongoing physician-patient relationship that begins with a First Month introductory engagement. The First Month includes:
The First Month is backed by the New Patient Guarantee, described in Section 6.
After the First Month, patients may continue with ongoing care (the Precision Partnership) at the rate of $500 per month, or end the engagement.
Our services are designed to complement your relationship with your primary care physician or other treating providers. They do not replace that relationship.
We do not provide emergency medical services, primary care, urgent care, or mental health services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
Our services are available to individuals who are:
We reserve the right to decline to establish or continue a patient relationship with any individual at our professional discretion.
If you are experiencing a medical emergency, including but not limited to chest pain, difficulty breathing, stroke symptoms, severe allergic reaction, or thoughts of harming yourself or others, call 911 or go to your nearest emergency room immediately. Do not attempt to contact us in an emergency.
A physician-patient relationship is established formally when we create your patient account in our electronic health record (Healthie). Browsing our website, submitting a contact form, scheduling a free Precision Call, or completing payment for the First Month does not by itself establish the physician-patient relationship.
Once the relationship is established, our services are advisory and optimization-focused in nature. Dr. Tagge will review your health data, order and interpret laboratory tests, and develop personalized health optimization protocols. He will not serve as your primary care provider or manage acute medical conditions outside the scope of concierge precision medicine.
Either party may end the patient-physician relationship at any time by giving notice via the patient portal or by emailing care@taggehealth.com. We will provide you with copies of any records generated during the engagement upon termination.
All fees for our services are disclosed at the time of enrollment.
Payment for the First Month of Concierge Precision Medicine ($500.00) is processed through Stripe at enrollment. Ongoing monthly fees for the Precision Partnership are processed monthly on the anniversary of your First Month enrollment date through our Healthie patient portal.
By enrolling in a program or membership, you authorize us to charge the applicable fees to your payment method.
All fees are charged in U.S. dollars. We do not bill insurance. Upon request, we may provide a superbill or receipt for you to submit to your insurance carrier or HSA/FSA administrator for potential reimbursement. We make no representations regarding what your insurer or plan administrator may or may not reimburse.
Your First Month of Concierge Precision Medicine is backed by the New Patient Guarantee.
If at any point during your thirty-day First Month engagement you determine that the Practice is not the right fit for you, you may request a full refund of the $500.00 First Month fee by emailing care@taggehealth.com. We will process the refund within seven business days.
The New Patient Guarantee applies only to the First Month engagement and only to patients who are new to the Practice. Refund requests received after the First Month has ended are not eligible.
We ask (but do not require) that you share the reason for your refund request, for purposes of practice improvement.
A refund of the First Month fee ends the patient-physician relationship. We will provide you with copies of any records generated during the engagement upon request.
Patients who continue into the Precision Partnership after the First Month are billed $500.00 monthly on the anniversary of their First Month enrollment date.
You may cancel the Precision Partnership at any time through the patient portal or by emailing care@taggehealth.com. Cancellation takes effect at the end of your current monthly billing cycle. You will retain access to Practice services through the end of that cycle.
Cancellation of the Precision Partnership ends the patient-physician relationship. We will provide you with copies of any records generated during the engagement upon request.
Laboratory testing fees are non-refundable once a lab requisition has been issued, as these represent direct costs passed through to laboratory partners.
By using our services, you acknowledge and agree that:
Dr. Tagge may, in his professional judgment, prescribe medications or order laboratory tests as part of your care plan. Prescriptions are issued through licensed pharmacy partners. Laboratory tests are ordered through Fullscript or direct lab accounts. You are responsible for any laboratory costs not covered by your program, which will be disclosed in advance.
We do not prescribe controlled substances as a standard component of our health optimization model. Any prescribing decisions are made solely at the professional discretion of Dr. Tagge.
Our recommendations are based on Dr. Tagge's clinical judgment and current medical evidence. Medicine is a practice, not a guarantee. We do not guarantee specific health outcomes.
Dr. Tagge responds to patient messages on business days, generally within one business day. We do not provide same-day response, after-hours access, or emergency communication. For urgent medical concerns, contact your primary care physician, an urgent care facility, or, in the case of emergency, call 911 or go to the nearest emergency room.
We comply with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. Our full Notice of Privacy Practices is available on this website.
Patient health information is stored in our electronic health record (Healthie), which has executed a Business Associate Agreement with us. Email communication outside the patient portal should be limited to non-clinical matters (scheduling, billing) to protect patient privacy.
