Privacy Policy

This Privacy Policy describes how Tagge Precision Health, PLLC ("Tagge Health," "we," "us," or "our"), operated by Daniel Garth Tagge, M.D., collects, uses, and protects information through our website at taggehealth.com and in providing our services.

It applies to website visitors, applicants, newsletter subscribers, and patients. How we use and disclose protected health information (PHI) under HIPAA is described in full in our Notice of Privacy Practices.

We do not sell, rent, or trade your personal or health information.

1. Information We Collect

A. Information You Provide

We collect information you choose to give us when you:

• Apply to work with Dr. Tagge through the application on our website
• Enroll in the Precision Health Assessment or Precision Partnership through our patient portal
• Subscribe to The Precision Health Report, our newsletter
• Contact us by email, phone, or text message

The application asks only for contact details and a few fixed choices. It does not ask for medical information, and you should not send any through it; that belongs in the patient portal after you are accepted.

B. Information Collected Automatically

We use Plausible Analytics to understand how our website is used. Plausible does not use cookies and does not collect personal data; it records page views, referrers, and conversion events such as button clicks. See Plausible's Privacy Policy.

We use Google Analytics 4 to measure how visitors reach our website from our advertising and to measure conversions such as submitting an application. Google Analytics uses cookies and collects usage data (pages viewed, approximate location, device information) as described in Google's Privacy Policy. No health information is sent to Google Analytics. You can opt out with the Google Analytics opt-out browser add-on.

When you first arrive from an advertisement or an outside link, we set one first-party cookie recording where the visit came from (such as the campaign name and landing page). It contains no personal or health information.

Our hosting provider may briefly retain technical request data (such as IP address and browser type) for security and uptime. It is not used for marketing or shared with third parties.

C. Patient Health Information

If you become a patient, we collect the health information needed to care for you, which may include medical history, medications and supplements, allergies, laboratory and imaging results, records from other clinicians (with your authorization), lifestyle and health goals, and wearable or home measurement data you choose to share. This information is protected health information governed by HIPAA and our Notice of Privacy Practices.

2. How We Use Information

• To review applications, respond to inquiries, and schedule care
• To provide clinical care and maintain an accurate longitudinal medical record
• To process payments and manage billing
• To communicate with you about your care, appointments, and results
• To send the newsletter and practice updates if you have subscribed
• To improve our website and services
• To meet legal and regulatory requirements

3. How We Use Technology and AI in Your Care

Dr. Tagge builds and uses software tools, including artificial intelligence, to organize patient records and prepare drafts of analyses and Precision Health Plans. For patients, this can include identifiable health records. Those records are processed only within OpenAI's HIPAA-compliant clinical environment, under a signed Business Associate Agreement, and are not used to train public AI models.

These tools support Dr. Tagge's work. They do not diagnose, treat, or make decisions. Dr. Tagge reviews everything they produce before it becomes part of your care, and every clinical decision is his.

4. Service Providers and Business Associates

We share information only with vendors that need it to support the practice. Every vendor that can access protected health information has signed a HIPAA Business Associate Agreement (BAA) with us.

• Healthie: electronic health record, scheduling, payment, telehealth video, and patient portal (BAA in place)
• Stripe: payment processing through Healthie (BAA in place)
• Google Workspace: business email and secure file storage (BAA in place)
• OpenAI: clinical AI tools within a HIPAA-compliant environment (BAA in place)
• A transactional email service that delivers website applications to Dr. Tagge (contact details only, no health information)
• Our newsletter provider, which holds subscriber email addresses and the page they subscribed from (no health information and no record of whether a subscriber is a patient)
• Plausible Analytics and Google Analytics: website measurement (no health information)

5. Other Disclosures

Patient information is shared with other clinicians, laboratories, and pharmacies involved in your care, and as you authorize. We may also disclose information when required by law (such as a court order, public health reporting, or oversight by the North Carolina Medical Board) or to prevent a serious threat to someone's health or safety. 6. Advertising, Lead Forms, and Email

We do not use the Meta Pixel or any tracking technology other than those described in Section 1. If you submit your name and email through a social media lead form (such as Facebook or Instagram Lead Ads) or our website, we use it only to deliver what you requested and to send health education and practice updates.

Newsletter and lead information is stored separately from protected health information, carries no record of whether you are a patient, and is not used for clinical care. You may unsubscribe from marketing email at any time with the link in any email or by writing to care@taggehealth.com. Appointment and care-related messages continue while you are an active patient.

7. Data Security

We use administrative, technical, and physical safeguards to protect your information, including encryption in transit and at rest, multi-factor authentication, and access limited to those who need it to care for you. No system is completely secure, and we cannot guarantee absolute security.

8. Breach Notification

If a breach affects your protected health information, we will notify you without unreasonable delay and no later than 60 days after discovering it, describing what happened, what information was involved, the steps you can take, and what we are doing in response.

9. Data Retention

Medical records are retained for at least the period required by North Carolina law and medical board guidance, and are then securely destroyed. Information about non-patients (such as applications and inquiries) is retained no longer than 3 years.

10. Your Rights

You may request access to or a copy of your information (in electronic form where available), correction of inaccurate information, deletion of non-medical information subject to legal retention duties, restrictions on certain uses, and an accounting of certain disclosures. You may opt out of marketing communications at any time. Your full HIPAA rights are listed in our Notice of Privacy Practices. To exercise any right, write to care@taggehealth.com.

11. Children's Privacy

Our services are for adults 18 and older. We do not knowingly collect information from anyone under 18, and we will delete any such information we learn we have collected.

12. Links to Other Websites

We are not responsible for the privacy practices of other websites. Please review their policies before giving them information.

13. Changes to This Policy

We may update this Privacy Policy. The revised policy will be posted here with a new effective date, and material changes affecting patients will be communicated in writing.
