Precision Partnership
Primary care and longevity with the same physician.
Precision Partnership combines comprehensive primary care with ongoing longevity-focused care. I handle what comes up day to day while also holding the long view of your health.
Precision Partnership is not open enrollment. Membership follows an initial Precision Health Assessment, based on mutual fit and practice capacity.
I keep this practice small so I can provide this level of attention consistently.
No waiting rooms. No rushed office visits. Care happens through home visits in the Charlotte area, secure virtual visits across North Carolina, and direct communication with Dr. Tagge.
What it includes
One relationship, and what it covers.
Comprehensive primary care
Routine and preventive care, evaluation of new concerns, and ongoing management of your health and medical conditions.
Longevity-focused care
A long-term strategy for preserving health, function, and resilience while identifying risks worth addressing earlier.
Four comprehensive reviews each year
Dedicated time roughly every three months to review what has changed, reassess priorities, and decide what comes next.
Individualized testing and interpretation
Testing selected purposefully, based on your history, physiology, goals, and the clinical questions we are trying to answer.
Direct physician access between scheduled reviews
Contact me directly for appropriate questions, new symptoms, medication issues, and other non-emergency concerns between scheduled reviews.
Medication management and care coordination
Prescribing and ongoing medication review, plus coordination with specialists, imaging, procedures, and other clinicians when appropriate.
How care unfolds
The shape of a year together.
Four comprehensive reviews anchor the year, alternating between your home and video. They set the rhythm of the relationship; they are not the whole of the care.
Start
Initial home visit
A comprehensive review: examination, priorities, and a baseline plan.
3 months
Virtual follow-up
Review progress, results, questions, and adjustments.
6 months
Home visit
Reassess your health: examination, testing, and the plan.
9 months
Virtual follow-up
Review progress and refine the next phase.
Year 2
The relationship continues
History, data, and the plan carry forward rather than starting over.
Care continues between visits. Direct physician communication, episodic medical care, medication management, results review, and follow-up happen throughout the year as needed.
After the Assessment
Two ways we can work together.
Precision Partnership
I am your doctor.
- Comprehensive primary care
- Longevity medicine
- Direct physician access
- Home visits in the Charlotte area
- Proactive quarterly care
Longevity Membership
I help manage your long-term health while you keep your primary care physician.
- Scheduled optimization reviews
- Longitudinal labs and ongoing interpretation
- Prevention planning
- Lifestyle and supplement guidance
- Defined messaging access
The Assessment does the sorting.
Which relationship fits is not a decision you have to make from a page. We make it together, after I know your history, your goals, and your data.
Ongoing membership
Ongoing care is available by membership after the initial Precision Health Assessment. Membership options begin at $3,000 per year, depending on the scope of care. Comprehensive concierge primary care and longevity services are discussed after the Assessment, so we can determine which level of support makes sense.
Precision Health Assessment · One assessment. One flat fee. Applied toward membership if you join within 30 days.
Questions
Practical questions.
You contact me directly by phone or email for questions, new symptoms, medication issues, results, and follow-up. Routine messages receive a personal response within one business day during published practice hours. This is direct access for medical care, not emergency coverage.

I keep this practice small. I read every application myself.
A small practice is what makes this level of attention possible. Accepted patients begin with the Precision Health Assessment, and we decide what comes after it together, once we both know it is the right fit.
