A ferritin of 20: what the lab calls normal, and what it is costing you.

A ferritin of 20 clears most labs' floor and explains a great deal of hair shedding, fatigue, and restless legs anyway. Two things get missed. Ferritin rises with inflammation and can hide a real deficiency, and in a man or a postmenopausal woman, low iron is bleeding until proven otherwise.

A ferritin of 20 ng/mL is inside the reference range at almost every lab in the country, so it comes back unflagged and nobody mentions it. It is also low enough to explain hair that sheds in the shower, legs that will not settle at night, a heart rate that climbs on one flight of stairs, and the kind of tiredness that sleep does not fix.

Both of those things are true at once, and the gap between them is the whole subject of this page.

Why the lab says normal

Ferritin's reference range is built to catch anemia. The bottom of it, often somewhere between 10 and 30 depending on the lab and your sex, is roughly the point below which your red blood cells start to suffer.

That is a real threshold and it is the wrong one for most people reading this. Long before your hemoglobin falls, your iron-dependent machinery starts running short. Iron is required for oxygen transport, yes, but also for the electron transport chain in every mitochondrion, for thyroid hormone conversion, for dopamine synthesis, and for the hair follicle, which is one of the most metabolically demanding tissues you own.

So you can have a perfectly normal complete blood count, no anemia at all, and still be functionally short of iron. The lab is answering "are you anemic." You are asking "why do I feel like this."

For hair, energy, mood, and exercise tolerance, I want ferritin above 70. For restless legs specifically, the threshold most people respond to is higher still.

The first thing that gets missed

Ferritin is an acute phase reactant. It goes up with inflammation, infection, obesity, liver disease, and alcohol.

Which means a ferritin that looks acceptable can be concealing a genuine deficiency underneath. A ferritin of 60 in someone with an hs-CRP of 6 is not the same as a ferritin of 60 in someone whose inflammation is quiet, and treating them as equivalent is a common error.

This is why ferritin alone is not enough. I want serum iron, TIBC, and transferrin saturation, plus an hs-CRP to know whether the ferritin can be taken at face value. A transferrin saturation under 20 percent points at deficiency regardless of what the ferritin says.

The thing that actually matters

Here is the part that turns this from an optimization question into a medical one.

In an adult man or a postmenopausal woman, iron deficiency is gastrointestinal blood loss until proven otherwise.

Men do not menstruate. Postmenopausal women no longer do. Iron does not leave the body in meaningful quantities by any other ordinary route. So if it is low, either it is not going in, or it is going out somewhere, and the somewhere that matters is the gut.

That warrants a real evaluation: celiac serology, because impaired absorption is a common and quiet cause, and in most cases endoscopy and colonoscopy. Sometimes the answer is a benign ulcer or an arteriovenous malformation. Sometimes it is a colon cancer found early enough to matter.

I labor this because the intuitive response to a low ferritin is to start iron, and iron will raise the number. The symptoms improve, the ferritin normalizes, and the reason it was low in the first place goes unexamined for another two years.

In a menstruating woman the calculus is different, and heavy periods are the usual and sufficient explanation. Even then it is worth asking how heavy, because a cycle severe enough to drive ferritin to 20 is itself worth treating.

What a chatbot does well, and where it stops

Ask a model about a ferritin of 20 and you will get a good answer. It will tell you the value is low-normal, explain that ferritin reflects iron stores, connect it to fatigue and hair loss, and usually suggest supplementing with vitamin C and rechecking. It may well mention that ferritin rises with inflammation. That is more than many people are told.

What it cannot do is order the transferrin saturation, take a menstrual history, examine you, or refer you for the colonoscopy that a low ferritin in a fifty-two-year-old man demands. It also cannot know what it was not shown, and in this presentation the thing it was not shown is usually the thing that matters.

What I would order

Ferritin with serum iron, TIBC, and transferrin saturation. An hs-CRP so I know whether to believe the ferritin. A CBC with indices, because the MCV tells me whether B12 or folate is in the picture too. Celiac serology in almost everyone with unexplained low iron. A full thyroid panel, because iron deficiency and impaired T4 to T3 conversion travel together and each makes the other harder to spot.

Then the history, which is the part no panel replaces. Cycle. Medications, particularly proton pump inhibitors and anti-inflammatories. Alcohol. Diet. Endurance training, which drives losses people do not expect. Blood donation, which is a genuinely common answer that nobody thinks to mention.

On actually correcting it

Two practical things, because they change how fast this resolves.

Oral iron is better absorbed on alternate days than daily. Taking it every day raises hepcidin, which then blocks the next dose, so more frequent dosing can absorb less total iron. Take it with vitamin C, away from calcium, coffee, and tea.

And recheck. Repletion takes months, not weeks, and the ferritin should be rising on a schedule you can see. If it is not, the assumption about where the iron was going was wrong, which is information.

The point

A ferritin of 20 is a normal result and a real problem, and holding both of those in mind at the same time is most of the job. The optimization question is why you feel the way you do. The medical question is where the iron went. The second one is the one that occasionally saves a life, and it is the one an unflagged lab result and a confident chatbot will both walk right past.
